Low age- and gender-adjusted ALP is a key factor for being able to diagnose HPP

The way to a prompt and correct diagnosis begins with a simple blood test. A blood test that reveals low levels of the enzyme alkaline phosphatase (ALP) can either support or eliminate an HPP diagnosis. Patients with hypophosphatasia generally have an ALP activity that is below the lowest normal level adjusted for age and gender. If low ALP activity is demonstrated, an HPP diagnosis can be determined by testing for elevated serum pyridoxal 5’ phosphate (PLP) or phosphoethanolamine (PEA) in the urine. An issue, however, is that many laboratories don't raise the red flag when confronted with low levels of ALP activity in a patient.

ALP-nivåer

The ALP values from paediatric patients are based on data from a Swedish child reference range project. For children younger than 6 months, data has been extrapolated from the CALIPER study. The reference values for the adult patients older than 18 years comes from the Nordic Reference Interval Project 2000 (NORIP). Data for newborn infants 7 days after birth is limited and the ALP activity can be variable in this population.
